There has been some pushback among hospital presidents for pushing physician behavior change. The group agreed that these changes are necessary to meet the cost constraints facing the entire healthcare industry. The group is amenable to splitting H&Ps from consults.
[] Anupam to determine if there is a workflow to generate a patient's FIN for elective cases before the patient arrives at the hospital for an outpatient procedure requiring an H&P.
The next CareNet+ iteration
There is a new version of CareNet+ that should be rolled out in the Fall. Tahir, Pranjal, Bob and Chris volunteered to pilot the new version and provide any feedback to the CareNet+ team.
CPOE
The group asked that Anupam reach out to Nursing to make sure the nursing staff is knowledgeable about how to use the PowerPlans once the physician signs the initial order.
ICD-10
Anupam presented the technical challenges for CareConnection migrating over to ICD-10:
* 240 users who have saved ICD-9 codes in their pre-completed PowerNotes will have an error when trying to save those notes after September 30, 2015.
* All ICD-9 diagnosis favorites will be wiped out on October 1, 2015.
Anupam provided the list of physicians who have been identified as having either a personal diagnosis favorite or a pre-completed note with an ICD-9 code that needs to updated. The site Clinical Informatics teams are working on updating the user's templates over the next three months. The physician has to physically log into their account with the analyst to make these changes.
Anupam encouraged the group to publicize the educational efforts including in-person sessions with an external documentation specialist, print materials (pocket cards, one-page pdfs) and the ICD-10 link within CareConnection.
Proposed 2016 Physician Informatics Targets
Anupam proposed two KRAs for 2016:
- Timely discharge including
- ePrescribing
- Discharge summary availability when the patient leaves the hospital
- Discharge instructions when the patient leaves the hospital
- Routing outstanding test results to the appropriate care team if those results become available after the patient has left the care setting
- Increasing PowerPlan utilization
Default discharge prescription number
The group was amenable to changing the default prescription number to seven days instead of 30 days.
Decision support for CT scans
To meet the CMS mandate to implement radiology decision support by January 1, 2017, Advocate will be working with Christ, Lutheran and Good Shepherd for a test of decision support for CT scans only.
Issues to track from prior meetings
[] Anupam to submit change request to withdraw "Reconcile and Plan" functionality.
[] Chris to distribute a discharge summary built from key events.
[] The CareNet+ upgrade has been delayed. There is no estimated time when the upgrade will be ready for testing.
[] If the signature + specialty request (with changing the resident's specialty with each rotation) and SmartTemplate work are performed by separate teams, then we will proceed down both paths simultaneously. Otherwise, we will work on the SmartTemplate request first before adding tropoinin in the daily labs SmartTemplate.
[] Anupam to update the website with tips & tricks on how to better use the documentation functionality within PC Touch
[] Anupam to ask Cerner for the estimated dates when H&P, operative notes and discharge summaries will be available in PC Touch (again)
[] Anupam to investigate options to pull in patient diagnoses on demand in PC Touch
[] Anupam is still waiting to hear back from the Advance Care Planning Council about changing the requirement for a physical signature for LET orders.
The next meeting is scheduled for August 13th at 0730 hours.
